By having a dentist perform a quick, in-office risk assessment, you may receive additional preventive benefits based on your oral health needs, helping to achieve better oral and overall health at no additional cost.
Enhanced benefits include:
- Cavity susceptibility test
- Up to 4 cleanings & fluoride treatments per 12 month period
- Annual nutrition/oral hygiene/tobacco cessation counseling
- Sealants once every 3 years
- Full mouth debridement
- Periodontal maintenance
Double-Up Max
Boost your annual benefit maximum by by accumulating $250 each year up to your calendar year maximum in bonus benefits for future coverage periods. When you’re faced with a dental procedure that exceeds the annual cap, those accrued carryover bucks can step in to bridge the gap.
