Leave of Absence

If you find yourself in a situation where you require time off to attend to your own well-being or that of a cherished family member, rest assured that our leave of absence programs are here to support you every step of the way. Should you need to take more than three consecutive business days away from work due to personal medical reasons, caregiving for a sick family member, military service, or to bond with a newborn, you’ll need to request a leave of absence.

Family or Medical Leave

The Family and Medical Leave Act (FMLA) provides eligible employees with up to twelve weeks of unpaid leave, ensuring job protection, during the need to care for their own or a family member’s serious health condition, in accordance with the criteria set by Federal and State leave regulations.

Please contact a member of the HR team to request a leave under the FMLA.

Medical Leave

A medical leave from work typically refers to a period of time when an employee is granted permission to be away from their job due to health-related issues. This could be due to the employee’s own medical condition, the need to care for a family member with a serious health condition, or for reasons related to pregnancy or childbirth.

The period of time an employee is on medical leave may run concurrently with time taken under the FMLA.

Through Diversified’s short term disability policy, approved medical leaves may be compensated for up to 8 weeks at 100% of salary and 4 weeks at 66.67% of salary.

Additional leave may be available as per state or local regulations.

Paid Parental Leave

Diversified’s paid parental leave policy runs concurrent with FMLA and provides 12 weeks paid at 100% for the birthing parent through a combination of our Short Term Disability and Parental leave policies.  Leave for the non-birthing parent provides up to 6 weeks paid at 100%, may be taken within 12 months following the birth or adoption of the child and may be taken intermittently.

Additional leave may be available as per state or local regulations.

For information on other types of leaves of absences, please visit your employee manual in UKG.

Requesting Leave

Employees must notify their supervisor and Human Resources at least 30 days’ prior to their intent to take leave or as soon as possible.  HR will provide the necessary paperwork and guidance to facilitate the leave process.  All information related to an employee’s leave will be treated with the utmost confidentiality.

Return to Work

Supervisors will work with you to ensure a smooth transition back to work.  Flexible return-to-work options can be discussed on a case-by-case basis with your supervisor as long as the request does not pose a hardship to the team/business and use of earned paid time may be required.
